UHS is currently making appointments for Pre-K and Kindergarten registration! Call (615) 735-1975 to make an appointment. Registration date is April 11, 2025.
Pre- K Students must be 4 years old on or before August 15, 2025 in order to start school for the 2025- 2026 school year.
Kindergarten Students must be 5 years old on or before August 15, 2025 in order to start school for the 2025-2026 school year.
It is very important to register for the school your child will be attending in the 2025-2026 school year.
Please bring the following information when registering your child:
Birth Certificate
Tennessee Certificate of Immunization
Physical Examination (conducted by August 1, 2025)
Proof of Smith County residency (utility or tax statement)
Proof of custody if the child does not live with one or both natural parents.
Students may register for Pre-K & Kindergarten without required documentation; however, students must have documentation to begin school in August, 2025
